The penis, the male sexual organ, is covered by a layer of foreskin. Many diseases are manifested on the penis and foreskin. Many men find hard, granular bumps under the epidermis of their penis, but they don't know what it is or what disease it is. In this case, can they have sex with their wives? Many diseases can cause hard, granular bumps under the epidermis of the penis. Let's learn about it together. Generally speaking, if the condition persists for a long time and there is no local change, it may be pearly papules or pseudo-warts. However, if the condition is short and there is a history of unclean sexual contact, the possibility of condyloma acuminatum cannot be ruled out. Pearly papules are a physiological developmental variation. They are neither tumors nor sexually transmitted diseases. Clinically, they are white, skin-colored or light red papules that do not fuse with each other and are arranged in one or several rows along the coronal groove at the posterior edge of the glans. Pseudo-warts. The main manifestation is a light red rash that looks like fish roe or velvet, with a smooth surface. There is a granular feeling after touching it. Generally, there are no subjective symptoms or only a mild itching sensation, which is easily confused with condyloma acuminatum. Condyloma acuminatum appears initially as small, moist papules of light red or dirty gray, which quickly develop into wart-like proliferations of varying sizes that may fuse and take the shape of cauliflower or cockscomb, sometimes with secretions and a foul odor. 1. Condyloma acuminatum 1. Condyloma acuminatum is a sexually transmitted disease caused by human papillomavirus (HPV) infection, with proliferative damage in the anal and genital areas as the main manifestation. It mostly occurs in young and middle-aged people aged 18 to 50. The disease occurs after a latent period of about half a month to eight months, with an average of three months. This disease is relatively common and is mainly transmitted through sexual contact. 2. Diagnosis 1). Typical skin lesions are papules in moist areas such as the genitals or perianal area, papillary, cauliflower-shaped or cockscomb-shaped fleshy growths with a rough, keratinized surface. 2). Auxiliary examinations: acetic acid white test is positive, nucleic acid hybridization can detect HPV-DNA related sequences, PCR test can detect specific HPV-DNA amplification bands, etc. 3) Most patients have a history of unclean sex or a history of infection from their spouse. A small number of condyloma acuminatum are infected through contact with contaminated utensils, and newborns can also be infected through the birth canal. The incubation period ranges from 1 to 8 months, with an average of 3 months. 3. Prevention 1). Resolutely put an end to sexual promiscuity Condyloma acuminatum patients are mainly infected through sexual contact. If one party in the family is infected from society, they will pass it on to their spouse through sexual life and to their family members through close contact, which will not only bring physical pain but also cause family discord. Therefore, improving sexual morality and not engaging in extramarital sex are important aspects of preventing the occurrence of condyloma acuminatum. Make sure that sexual partners also receive diagnosis and treatment. 2). Prevent contact infection and pay attention to personal hygiene Do not use other people's underwear, swimsuits and bathtubs; do not take a bath in a public bath, showering is recommended, and do not sit directly on the bathtub seat after bathing; use squat toilets as much as possible in public toilets; wash hands with soap before and after going to the toilet. 2. Pearl rash 1. Pearly rash, also known as penile pearly papules, coronal sulcus or glans papules, is also called hairy penis or multi-papillary penis by some scholars. Currently, most scholars believe that this is a normal physiological variation, so it will not affect health and will not be infected or spread through sexual intercourse. 2. Clinical manifestations and diagnosis Pearly penile papules are more common in men around 20-30 years old. The damage mainly occurs at the junction of the edge of the glans penis and the coronal sulcus and/or the frenulum. The lesions are 1-3mm in size, and the lesions are larger on both sides of the frenulum. The top of the papules is round and smooth, and some papules are hairy or thread-like. The papules are mostly not fused with each other, and are densely arranged in one or more rows. They are obvious on the dorsal side of the glans penis and may partially or completely surround the glans penis. The color of the lesions is mostly pearly white, a few are light red or skin-colored, and some may appear mildly red and swollen. There is no tenderness, no rupture, and the patient has no subjective symptoms. The key points in differentiating and diagnosing penile pearly papules and condylar warts are that in the former, the lesions are limited to the edge of the glans, the number of lesions is larger, they are pearly milky white papules with a smooth surface, the lesions do not increase in size as the disease progresses, and the acetic acid test is negative; while in the latter, the lesions are not limited to the edge of the glans, they are mostly single and scattered, the lesions are larger, the surface is not smooth, and they are mostly cauliflower-shaped, they gradually increase in size as the disease progresses, the acetic acid test is positive, and concave cells are found in histopathological examination. 3. Pseudocondyloma acuminatum 1. Pseudocondyloma acuminatum is a pseudocondyloma acuminatum, which should be diagnosed with caution. This disease is a benign papilloma, not a sexually transmitted disease, and is not contagious. Most of the 1-2 mm smooth papules are seen on the inner side of the labia minora of women aged 20 to 30, clustered but not fused with each other. Pseudocondyloma acuminatum is roe-like or small polyp-like and sometimes itchy. It may be caused by physiological variation or vaginal Candida albicans causing increased leucorrhea and irritation. 2. Treatment Pseudocondyloma acuminata has no adverse effect on health and the rash is self-limited, so generally no treatment is required. |
