What causes lower body pain?

Many women will not experience lower body pain for no reason. When there is vaginal pain, it can mostly be considered to be caused by inflammation, environmental factors or local injuries. Vaginal pain cannot be ruled out as being caused by first sexual contact. It can be mainly judged by several factors, including physiological factors, psychological factors, and traumatic factors. Timely examination and diagnosis should be carried out, and appropriate methods should be chosen to deal with it.

1. Pathological causes

Vaginal spasm is divided into four types: primary, secondary, complete and situational. Primary vaginitis accounts for the majority, which means that from the beginning of sexual intercourse, vaginal spasm occurs when sexual intercourse occurs, and they have never experienced successful sexual intercourse. Secondary vaginitis refers to a history of normal sexual intercourse, and then vaginal spasm occurs and sexual intercourse cannot be completed. Complete vaginitis means that penetration cannot be achieved under any circumstances and sexual intercourse cannot be completed. Situational vaginitis means that penetration can be achieved sometimes and not sometimes, such as a doctor can successfully perform a gynecological examination, but penetration cannot be achieved during sexual intercourse.

2. Psychological factors

(1) Family education factors: Patients may have been influenced by traditional sexual concepts such as feudalism and religion during childhood or adolescence, and regard sexual life as obscene, evil and lewd behavior. They believe that the most important thing for daughters is to maintain chastity, and that losing chastity will ruin their reputation. If women show interest in sexual activities, they are considered to be immoral. As a result, they consciously or unconsciously associate sexual activities with shame, humiliation, anxiety, fear and guilt. A study in Ireland pointed out that among 53 patients with vaginal spasm, 18 cases (34%) were affected by negative education, while the control group (66 cases) only accounted for 29%.

(2) Traumatic factors: Having suffered pain during sexual intercourse has left an indelible fear memory, forming a negative conditioned reflex. When the woman senses that sexual intercourse is about to occur or has engaged in sexual intercourse, she will experience vaginal spasm due to fear. In the aforementioned Irish data, among the 53 cases of vaginal spasm, 24 had been sexually assaulted, accounting for 27%. Among the 66 cases in the control group, 6 had been assaulted, accounting for 9%, which is a significant difference.

(3) Disharmony in the marital relationship: When the husband first married, his sexual behavior was rough, causing severe pain and trauma, which made the woman afraid of sexual intercourse. On the other hand, disharmony in the marital relationship or incorrect sexual techniques caused the woman to become irritable, angry or fearful, resulting in vaginal spasm.

2. Organic factors: Any gynecological disease that can cause pain during sexual intercourse can lead to vaginal spasm, such as thick and tough hymen, narrow vaginal scars, vulvitis combined with ulcers of the vulva or vestibule, endometriosis, and senile atrophic vaginitis.
