It can be mainly divided into the following points:

(1) Psychological (psychogenic, mental): Psychological impotence accounts for about 85-90% of all impotence cases and is the most common physiological dysfunction disease. Examinations show that the patient does not have an organic disease that causes physiological dysfunction, but the penis cannot erect during sexual intercourse. However, the penis can erect in some non-sexual activities, such as in dreams, when reading some sexually stimulating books and magazines, or when the bladder is full of urine, or even during masturbation. The mechanism of psychological impotence may be due to a variety of mental and psychological factors interfering with the brain's sexual center, so that the brain's sexual nerve center cannot get enough excitement. There are many psychological factors that cause impotence, which will be discussed in detail in the following content.

(2) Organic: Although organic impotence only accounts for about 10-15% of the total number of impotence, it has many causes and complex pathology. Generally speaking, it is caused by congenital abnormalities, diseases, drug effects, and iatrogenic factors.

① Urogenital malformations: congenital penile curvature, double penis, small penis, penoscrotal displacement, retroverted bladder, urethral fissure, congenital testicular absence or dysplasia, corpus cavernosum fibrous scar formation, varicocele, etc. may cause erection failure due to deformity, curvature, corpus cavernosum dysfunction, etc.

② Urogenital diseases: Impotence is common in patients with chronic urogenital inflammation, such as orchitis, epididymitis, urethritis, cystitis, prostatitis, etc., among which chronic prostatitis is the most common cause of impotence. Urogenital surgery and certain injuries, such as prostatic hyperplasia, prostatectomy, urethral fissure, penis and testicular injury, etc. can all cause impotence. Patients with chronic renal failure often suffer from impotence due to testicular atrophy and decreased testosterone.

③ Endocrine diseases: Impotence is often caused by endocrine diseases, mainly diabetes, hypothalamic-pituitary abnormalities and primary gonadal insufficiency. According to foreign reports, about 23% to 60% of male diabetic patients suffer from varying degrees of impotence. The mechanism of its occurrence is mainly related to the autonomic neurofibropathic changes on the corpus cavernosum of the penis, stenosis of the penile blood vessels, endocrine abnormalities and mental factors.

④ Neuropsychiatric diseases: sequelae of stroke, cranial injury, cerebral palsy, myasthenia gravis, late-stage M_D, spinal cord injury, paralysis, multiple sclerosis, lumbar disc herniation, chronic alcoholism, etc. can all lead to impotence. Mental retardation, schizophrenia, neurosis, depression, epilepsy, etc. can also cause impotence.

⑤ Cardiovascular disease and drug effects can also lead to impotence. For example, antihypertensive drugs such as methyldopa, reserpine, phenothiazines, cimetidine, metoclopramide, tricyclic antidepressants and hormone preparations (estrogen, progesterone) all have this effect.

(3) Iatrogenic: Impotence caused by nerve damage due to surgery or the use of certain drugs. The above is the relevant introduction.
