Drug therapy

1. Podophyllum resin: This treatment is suitable for wet warts in moist areas, such as wet warts on the glans penis with excessive foreskin but no circumcision. Apply 20% podophyllum resin tincture to the skin lesions or before using the medicine, first use oily antibacterial ointment to protect the normal skin or mucous membrane around the skin lesions, then apply the medicine, wash with 30% boric acid water or soapy water 4-6 hours after use, and repeat the medicine after 3 days if necessary. This medicine is the first medicine used in foreign countries to treat this disease, and it can usually be cured after one use. However, it has many disadvantages, such as being highly destructive to tissues and causing local ulcers if used improperly. It is highly toxic, with the main symptoms being nausea, intestinal obstruction, leukopenia and thrombocytopenia, tachycardia, and auria or oliguria. Therefore, it must be used with caution. If the above reactions are detected, the drug should be discontinued immediately.

2. Virus-killing drugs: Currently, they are mainly based on macrocyclic triterpenes. The main principle of their treatment is that macrocyclic triterpenes can kill HPV viruses hidden in the skin and mucous membranes. This type of treatment has a long course of treatment, and the warts fall off slowly, but the recurrence rate is very low. The advantages of this type of drug treatment are that it has very little side effects, is very safe, will not cause skin damage to normal skin, and will not leave scars after treatment. It is a good choice for condyloma acuminatum in the male urethra, mouth, and rectum.

3. Corrosive agent or disinfectant: 30%-50% trichloroacetic acid or saturated dichloroacetic acid, or 18% peracetic acid are commonly used. Use 10% salicylic acid glacial acetic acid or 40% formaldehyde, 2% liquefied phenol, 75% ethanol distilled water 100ml mixed solution, apply it locally, for glans penis, perianal wet warts, once a day or every other day, the effect is very good. Disinfectant can be applied externally with 20% iodine tincture, or 2.5-5% iodine tincture can be injected into the base of the wart, 0.1-1.5ml each time, or use new gentamicin externally or 0.1-0.2% externally, the latter needs to be combined with systemic therapy.

4. Anticancer drugs

(1) 5-Fluorouracil (5-Fu): Generally, 5% ointment or cream is applied externally, twice a day, for a course of 3 weeks. 2.5%~5% fluorouracil wet compress is used to treat penile and perianal condyloma acuminatum, applied for 20 minutes each time, once a day, for 6 times as a course of treatment. It can also be made into suppositories using polyethylene glycol as a base and adding 5% 5-Fu powder to its dry matter to treat male urethral condyloma acuminatum. It can also be injected with 5-Fu base, and more can be injected in batches.

(2) Thiotepa: Mainly used for urethral condylar warts that have failed to be treated with 5-Fu. Use suppositories (each containing 15 mg) daily for 8 consecutive days. You can also add 60 mg of this product to 10-15 ml of disinfected water and drip it into the urethra every week for half an hour. Side effects include urethritis. You can also use 10 mg of this product to 10 ml to soak the affected area 3 times a day, half an hour each time, to treat male penis and glans coronal sulcus warts. It is mainly used for those who still have residual warts or recurrence after treatment with other methods. You can also dilute this solution twice and soak the local area to prevent recurrence.

(3) Colchicine: 2-8% saline solution can be applied externally, twice, 72 hours apart, to treat penile warts. Superficial erosion may occur after application.

There are many kinds of drugs for treating herpes and warts, and their treatment mechanisms are different. Improper use will increase the burden on the liver, so they should be used selectively. When using antiviral drugs, you should also pay attention to identifying the side effects that lead to liver and kidney dysfunction.
