The most common side effects of PDE5 inhibitors are headache, flushing, indigestion and nasal congestion, all of which work through the nitric oxide-cyclic guanidine phosphate (NO-cGMP) pathway. Their contraindications are patients taking long-acting or short-acting nitrates. The following patients may be at risk for taking PDE5 inhibitors: active coronary heart disease, congenital heart disease with heart failure, hypotension, significantly enlarged heart, use of combination drugs to fight hypertension, and use of drugs that extend the half-life of PDE5.

Second line treatment

Vacuum negative pressure device therapy: When using a negative pressure device, a negative pressure ring is placed at the base of the penis to prevent blood from flowing back. The negative pressure acts on the cavernous body of the penis, attracting blood into the penis, causing the penis to passively erect. Generally, young patients are not easy to accept negative pressure devices. Elderly patients with erectile dysfunction who have a low frequency of sexual intercourse and patients who prefer non-invasive, non-drug treatments are more likely to accept negative pressure therapy. Side effects include penile pain, numbness, and inability to ejaculate.

Penile corpus cavernosum drug injection therapy: Injection of vasodilators such as prostaglandin E1, poplar alkaloids, and phentolamine into the penile corpus cavernosum can induce penile erection through local vasodilation. However, the dosage and injection method of the drug must be determined under the guidance of a doctor to avoid serious complications. The clinical efficacy of penile corpus cavernosum drug injection therapy is 70-80%, and adverse reactions include prolonged erection or persistent abnormal erection, penile pain, or fibrosis of the corpus cavernosum. If the penis continues to be erect for more than 4 hours, the patient should be treated immediately by the urology department for emergency treatment.
