The affected area loses its normal elasticity and atrophies over time. 2. Other symptoms If the disease invades the outer foreskin and scrotum, scaly erythema may be seen. If the navicular fossa is involved, frequent urination and dysuria may occur. Local burning sensation and itching may occur. Occasionally, fulminant edematous balanitis may occur, mainly manifested by obvious edema and severe itching of the foreskin of the penis, with superficial ulcers. 3. Diagnostic Basis 1. Acute superficial balanitis (1) Skin lesions are characterized by local erythema, swelling, erosion, exudation, and, in severe cases, blisters may appear. (2) It is often caused by local friction and poor rotation of the foreskin. External medications such as condom allergies can also cause acute inflammation of the glans foreskin. 2. Candidal balanitis (1) Skin lesions present as erythema, papules or papulorum, followed by flaky erosions with white, dotted, cheesy secretions on the surface that can be easily scraped off. (2) Microscopic examination and culture of local secretions revealed Candida albicans. 3. Cytoplasmic balanoposthitis (1) Localized infiltrative dark red spots occur on the glans penis, with a smooth surface and a little desquamation. (2) Histopathology has diagnostic value: it is characterized by epidermal hyperplasia and a large number of plasma cell infiltration in the dermis. 4. Mica-like and keratotic pseudoepitheliomatous glans balanoposthitis (1) Skin lesions are characterized by excessive keratinization or infiltration and hypertrophy, mica-like crusts, poor elasticity, and atrophic changes over time. (2) Histopathological changes include hyperkeratosis and pseudoepitheliomatous hyperplasia. Infiltration of dermal inflammatory cells is observed. |
