How to check male urethritis?

Routine urine examination shows an increase in white blood cell count or purulent urine, accompanied by an increase in red blood cells, and a small number of cases show macroscopic hematuria. The three-cup urine test shows a large number of pus cells and red blood cells in the first cup, while the second and third cups are basically normal. The number of bacteria in the first-stream urine cell culture is significantly higher than that in the midstream urine. In the urethral or vaginal secretion smear examination, gonococcal urethritis may show intracellular or extracellular gonococci. Non-specific urethritis can be cultured with secretions or anterior urethral swabs, showing a large number of bacterial growth. If no bacteria are found in the secretion smear and culture, there is a possibility of mycoplasma or chlamydia infection, and special methods of culture or PCR examination can be performed.

Chronic urethritis requires urethrocystoscopy to clarify the cause of the disease. Sometimes a metal urethral probe can be used to test whether there is stricture in the urethra. Urethral angiography can be performed when necessary. In the acute stage, the use of instruments to examine the urethra should be avoided.

Differential diagnosis

1. Acute cystitis

The main symptoms are bladder irritation symptoms such as frequent urination, urgency, and pain during urination. However, patients with cystitis mainly experience pain at the end of urination, and bacterial growth is found in midstream urine culture.

2. Acute pyelonephritis

The main symptoms are sudden urinary frequency, urgency, pain and other urinary tract irritation symptoms, often accompanied by low back pain, chills, fever and other symptoms. Physical examination shows percussion pain in the kidney area, and routine urine examination shows pus cells.

3. Acute prostatitis

Symptoms include frequent urination, urgency and pain when urinating, but prostatitis also causes perineal discomfort, difficulty urinating and fever. Rectal examination reveals enlarged prostate with tenderness.

4. Gonococcal urethritis

Symptoms include frequent urination, urgency and pain when urinating, redness and swelling at the urethral opening, thin or purulent secretions, and often a history of unclean sexual intercourse. A smear test of urethral secretions may reveal gonococci, which can provide a clear diagnosis.

5. Bladder tuberculosis

Symptoms include frequent urination, urgency, and pain during urination. Pus cells are found in the urine. There is often a history of urinary tuberculosis, and acid-fast bacteria can be found in urine acid-fast staining.

6. Trichomonas urethritis

Symptoms include frequent urination, urgent urination, burning pain and itching in the urethra, and trichomonas can be found in urethral secretions.
