1. Introduction to the Sperm Hypoosmotic Swelling Test

The sperm hypoosmotic swelling test, also known as the sperm tail hypoosmotic swelling test (HOST), was created by Jeyendran in 1984 after he found that the tail of human sperm exhibited different types of swelling in a hypoosmotic solution. This phenomenon reflects the physiological function of the sperm membrane and is related to sperm energy metabolism, fertilization, capacitation, apicosome reaction, and the fusion of sperm and egg membranes. The sperm tail is the sperm's movement organ. After the sperm is ejaculated into the vagina, it ascends through the cervix and uterine body, and finally reaches the fallopian tube and combines with the egg cell. To complete the above movement, a well-functioning sperm tail is required. Clinically, the survival rate of sperm can be evaluated by testing the integrity of the sperm membrane. When sperm is in a hypotonic solution, water molecules can enter the sperm through the sperm membrane, causing the sperm to increase in size and swell, especially the tail of normal sperm. Sperm with incomplete membranes or dead sperm generally do not swell.

The morphological integrity and functional activity of sperm membrane are important factors in the fertilization process. The sperm hypoosmotic swelling test is an in vitro test method that can evaluate the functional integrity of sperm membrane and is an effective method to evaluate sperm fertility.

Routine semen analysis, such as sperm density, motility and morphological examination indicators, is sometimes not enough to fully identify the cause of infertility. Therefore, clinical research on sperm function, such as sperm membrane function, has become increasingly important. Sperm membrane function is closely related to sperm metabolism, acrosome reaction, sperm capacitation and sperm-egg fusion, and has important guiding significance for the diagnosis and treatment of clinical male infertility.
