White discharge on men's underwear

Many middle-aged men report that there is white discharge on their underwear. This is probably due to insufficient kidney qi leading to spontaneous leakage of semen, or due to urinary tract inflammation and exudation caused by urinary tract infection. The three main symptoms of urinary tract infection are frequent urination, urgent urination, and painful urination, which are all abnormal changes in urination.

Frequent urination is actually a relative concept. Generally speaking, a normal adult urinates 4-6 times during the day and 0-2 times at night. A significant increase in the number of urinations is called frequent urination. Frequent urination can be caused by many reasons. Under physiological conditions, such as drinking a lot of water, the urine volume increases, and the number of urinations also increases; under pathological conditions, if the number of urinations increases and the urine volume is normal each time, the total daily urine volume increases, which is seen in diabetes, diabetes insipidus, etc.; the number of urinations increases but the urine volume decreases each time, or there is only the urge to urinate but no urine is discharged, which is seen in irritation of the bladder or urethra, such as inflammation or stones; obstruction of the lower urinary tract, such as prostatic hyperplasia, urethral stenosis, etc., can also cause frequent urination, usually accompanied by difficulty urinating, labored urination, shortened range, interrupted urine stream or dripping.

Urinary urgency refers to the inability to control urination. When the urge to urinate comes, you need to urinate immediately; or after urinating, you feel the urge to urinate again and need to urinate urgently. If you do not urinate in time, your underwear will be wet. Urinary urgency is generally seen in inflammation, stones, foreign bodies, tumors, urine concentration caused by high temperature environment, neurogenic bladder or mental factors.

Dysuria is the most direct reaction of inflammation and is usually severe. In medicine, it refers to pain in the urethra or the suprapubic area or perineum when the patient urinates. The pain can be mild or severe, often burning, and in severe cases, it can be as painful as a knife. Based on the characteristics of dysuria, different types of infection can be easily identified. If there is obvious pain at the beginning of urination or dysuria, the lesions are mostly in the urethra, which is common in acute urethritis; if there is pain at the end of urination and urgency, the lesions are mostly in the bladder, which is common in acute cystitis; if there is obvious pain at the end of urination, the pain is still felt after urination, or there is a feeling of "empty pain", or the pain is even without urination, the lesions are mostly in the urethra or adjacent organs, such as prostatitis; sudden interruption of urination accompanied by pain is seen in bladder, urethral stones or urinary foreign bodies; difficult urination accompanied by bloating and pain often indicates prostate hyperplasia in elderly men, and can also be seen in urethral stones; stinging or burning pain when urinating is often seen in acute inflammatory stimulation, such as acute urethritis, cystitis, and prostatitis.
