Pain between anus and scrotum

This line, together with the fibrous longitudinal diaphragm in the middle of the perineum, has the function of stabilizing and maintaining the normal position of the anus and rectum. Therefore, this part is also a relatively important part of the body surface. Therefore, pain in this area is generally considered to be a manifestation of local infection. The soft tissue between the base of the penis and the anus is called the perineum. Scrotal pruritus refers to abnormal itching of the scrotal skin. This syndrome has different names in ancient medical books. "Treatise on the Causes and Symptoms of Various Diseases" calls it "wet itching under the genitals", "The Authentic Treatise on Surgery" and "The Great Compendium of Surgery" call it "scrotal wind", and "Golden Mirror of the Medical Tradition. Essential Skills of Surgery" calls it "scrotal wind". In mild cases, only scrotal pruritus occurs. In severe cases, the scrotal skin becomes thicker, or yellow fluid oozes out and scabs form due to scratching.

Common symptoms

Itching of the scrotum due to damp-heat accumulation: At first, the genitals are extremely dry and itchy, and the patient likes to bathe in hot water. In severe cases, lumps may appear, shaped like red millet, with abnormal numbness and itching. When scratched, the scrotum will be soaked with water and the pain will be like burning fire. The tongue coating is yellow and greasy, and the pulse is stringy, slippery and rapid.

Itching of the scrotum due to deficiency of Yin and dryness of blood; itching of the scrotum, extremely itchy, restless when sitting or lying down, rough and thickened scrotal skin, bleeding from scratching, blood scabs from scratches, accompanied by thirst, irritability, red tongue, and weak and rapid pulse.

Itching of scrotum due to cold and dampness in the lower abdomen: The scrotum is damp and the itching is mild, accompanied by soreness and weakness in the waist and knees, bloating in the lower abdomen, difficulty in urination, swelling and heaviness of the lower limbs, white and moist tongue coating, and slow and weak pulse.

When drinking, staying up late, or having a poor physical condition, some people may suddenly feel slight pain inside the anus, a feeling of bloating, and discomfort with bowel movements. Then a lump will form and swell next to the anus, and the pain will gradually increase. Even coughing will affect the anus. Sometimes the fever may reach 38°C, or even a high fever. The pain will exist for 24 hours without any interval and has nothing to do with bowel movements. This is most likely perianal abscess.

Perianal abscess is an anal disease caused by pathogenic bacteria in feces entering the anal pits on the dentate line between the rectum and the anus, causing inflammation and suppuration of the anal glands inside. The infection is not limited to the anal glands, but will further spread to the soft fat tissue between the muscles around the anus, like a mouse digging a hole, creating a purulent tunnel. This state of pus accumulation around the anus is called perianal abscess.

When the perianal abscess matures, it will naturally rupture and discharge pus, and the pain will quickly ease, but this process will take a long time, and before the abscess ruptures, the pus will spread to the surrounding area, causing the condition to worsen. However, if you ask a doctor to cut it open as soon as the lump becomes soft (indicating that it has become abscess), you will quickly feel comfortable, the pain will be shortened, and the lesion will be relatively small. If it is an ordinary abscess in other parts of the body, it can be cured about 10 days after the abscess appears. But if it is perianal abscess, it is not so simple, because after the pus is drained, the purulent cavity shrinks and becomes smaller, but the purulent tunnel and fistula duct with the anal fossa as the primary focus still remain. Due to the characteristics of the long and narrow curved drainage, it is difficult to completely eliminate inflammation by incision drainage and the use of anti-inflammatory drugs. After the pus is drained, the outer wound surface of the abscess will heal temporarily, but soon when the inflammatory secretions in the anal fossa fill the fistula duct, it will be discharged from the original fistula mouth, and then anal fistula will be formed. Generally, most perianal abscesses will form anal fistula 2 to 3 months after the pus is drained. In some anal fistulas, the pus does not flow out from the rupture of the perianal skin, but is discharged directly into the rectum from the anal fossa of the primary lesion. This type of anal fistula is called internal fistula.

It can be seen from this that the incision and drainage treatment method can achieve good results for ordinary abscesses, but for perianal abscesses, it can only eliminate temporary pain. If the goal of radical cure is to be achieved, efforts must be made on the primary lesion.
