With urine in the bladder, nerve signals from the sacral nerves tell the muscle that wraps around the bladder, the detrusor, to contract to start the urination process. At the same time, nerve signals tell the urethra to open to allow urine to flow out. Research has shown that many factors may cause overactive bladder, such as the sacral nerves giving the bladder detrusor muscle a signal to contract when the bladder is not full (commonly known as unstable detrusor contraction).
