Normal semen is milky white and does not contain blood. If semen contains blood, it is called hematospermia.

Clinical manifestations

Hematospermia is characterized by blood in the semen during ejaculation, which may be accompanied by ejaculation pain, loss of libido, premature ejaculation, perineal pain and discomfort, initial or final hematuria after ejaculation, bladder irritation, etc. The characteristics of functional hematospermia are: if there is no ejaculation for a long time, hematospermia is likely to appear once ejaculation occurs, without other symptoms, and the duration is short and it is easy to relapse.

Due to the different causes, locations, amounts and duration of bleeding, the appearance of hematospermia is also different: hematospermia caused by inflammation and injury is evenly mixed; when it comes from bleeding in the urethral mucosa, it is manifested as bright red blood mixed in the semen, which is not mixed with the semen; if the bleeding is stored in the seminal vesicle for a long time, it is often brown; if it is fresh bleeding, it is pink. The vast majority of patients with hematospermia have a small amount of bleeding, which can be the initial, final or whole process of hematospermia.

Including urinalysis, urine bacterial culture and drug sensitivity test and chlamydia detection. Routine semen or prostatic fluid examination shows a significant increase in red blood cell and white blood cell counts. If necessary, semen bacterial culture plus drug sensitivity test can be performed. The use of PCR technology can confirm the diagnosis of tuberculosis. People over 40 years old should also check serum prostate specific antigen (PSA). In addition, routine blood tests, liver function, kidney function, coagulation time and electrolyte tests are performed to exclude hematospermia caused by chronic diseases and bleeding constitution.

2. Imaging examination

(1) Transrectal ultrasound is the preferred method for detecting hematospermia. It can also be used to guide seminal vesicle aspiration and biopsy or prostate biopsy to further clarify the cause of bleeding.

(2) The biggest advantage of magnetic resonance imaging (MRI) is that it can directly show bleeding in the seminal vesicle or prostate. MRI's three-dimensional cross-sectional imaging is considered the "gold standard" for imaging the body's gonads, accessory glands, and their ducts.

(3) Endoscopic examination: When hematospermia is suspected to be caused by lesions in the urethra, bladder, ejaculatory duct, or seminal vesicle, cystourethroscopy, ureteroscopy, or seminal vesiclescopy can be performed.

The commonly used abdominal plain film, excretory urography, vasectomy or seminal vesiculography can provide limited data, but are rarely used in the diagnosis of hematospermia. CT is not superior to transrectal ultrasound and MRI.

Diagnosis

The diagnosis of hematospermia is determined based on medical history, systemic examination, and examination of the urinary and reproductive systems. When making a diagnosis, be careful to exclude the possibility of bleeding from the sexual partner. It should be distinguished from the extremely rare melanospermia. The latter is a malignant melanoma that occurs in the prostate, seminal vesicles, or urogenital tract, characterized by dark brown semen or dark brown spots in the semen.

From the above, we know that blood in the ejaculation may be caused by hematospermia, which will seriously affect the patient's body. This must be taken seriously by the patient, and go to the hospital for examination as soon as it is discovered. It is best if there is no problem.
